Neurological varicella

Overview

Neurological manifestations of varicella (chickenpox) primarily occur due to direct viral invasion or as postinfectious immune-mediated complications, affecting the central nervous system (CNS). These complications can range from mild encephalitis to more severe conditions like cerebellar ataxia and transverse myelitis. 2139

Diagnosis

  • Clinical Presentation: Fever, headache, altered mental status, seizures, focal neurological deficits. 21
  • Imaging: MRI or CT scans to identify structural abnormalities such as encephalitis or demyelination. 39
  • CSF Analysis: Elevated white blood cell count, protein levels, and sometimes viral DNA detection. 21
  • Electroencephalogram (EEG): Abnormal patterns indicative of encephalitis. 21
  • Serological Testing: Confirm varicella zoster virus (VZV) infection through IgM and IgG antibodies. 21
  • Management

  • Antiviral Therapy: Initiate acyclovir (500-1000 mg/m2/dose IV every 8 hours) for suspected or confirmed VZV encephalitis. 21
  • Supportive Care: Management of seizures, intracranial pressure, and respiratory support as needed. 21
  • Monitoring: Close neurological monitoring, including serial assessments of mental status and motor function. 21
