Inflammatory bowel disease

Last edited: 4/23/2026

Overview

Inflammatory bowel disease (IBD) encompasses Crohn's disease (CD) and ulcerative colitis (UC), characterized by chronic inflammation of the gastrointestinal tract leading to symptoms such as abdominal pain, diarrhea, and extraintestinal manifestations. 13

Diagnosis

  • Clinical Presentation: Abdominal pain, diarrhea, rectal bleeding, weight loss.
  • Laboratory Tests: Elevated inflammatory markers (CRP, ESR), fecal calprotectin.
  • Endoscopy: Essential for diagnosis; mucosal appearance guides differentiation between CD and UC. 2930
  • Imaging: CT enterography, MRI enterography for CD; colonoscopy with biopsies for histopathological confirmation. 29
  • Genetic Testing: Not routinely required but may aid in diagnosis in specific cases. 4
  • Management

  • First-Line Treatments:
  • - 5-ASA Agents: For mild to moderate UC (e.g., mesalamine). - Antibiotics: For CD complications like fistulas (e.g., metronidazole, ciprofloxacin). - Aminosalicylates: For mild UC flares.
  • Advanced Therapies:
  • - Immunomodulators: Azathioprine, 6-mercaptopurine for maintenance of remission. - Biologics: Anti-TNF agents (e.g., infliximab, adalimumab), integrin receptor antagonists (e.g., vedolizumab), JAK inhibitors (e.g., tofacitinib).
  • Therapeutic Endoscopy: Considered for specific complications like strictures, bleeding, and fistulas. 6
  • Special Populations

  • Pregnancy: Maintain remission with thiopurines or avoid biologics; consider switching to methotrexate or budesonide if necessary. 434
  • Pediatrics: Focus on growth monitoring, tailored therapy (e.g., biologic use with caution), and therapeutic drug monitoring. 728
  • Elderly: Consider polypharmacy risks, efficacy of biologics, and potential for increased adverse events; prioritize treat-to-target strategies. 6263
  • Comorbidities: Increased vigilance for thromboembolic events, metabolic dysfunction-associated steatotic liver disease, and arthritis. 1751
  • Key Recommendations

  • Treat-to-Target Strategy: Aim for mucosal healing and symptom control using a combination of pharmacological and endoscopic monitoring. (Evidence: Strong 8)
  • Thromboprophylaxis: Consider prophylactic anticoagulation in high-risk IBD patients, especially those with prior thromboembolic events. (Evidence: Moderate 17)
  • Nutritional Support: Regular assessment and intervention for nutritional deficiencies, particularly vitamin D and micronutrients, in IBD patients. (Evidence: Moderate 69)
  • Adherence to Therapy: Enhance patient education and support to improve adherence, especially with complex regimens involving biologics and immunomodulators. (Evidence: Expert opinion 22)
  • Transition Care: Ensure structured transition programs for pediatric IBD patients moving to adult care, focusing on continuity of care and patient education. (Evidence: Expert opinion 33)
