Aortic valve cusp prolapse

Last edited: 4/10/2026

Overview

Aortic valve cusp prolapse refers to the abnormal bulging or protrusion of one or more aortic valve cusps. This condition can lead to aortic regurgitation or, in the context of transcatheter aortic valve implantation (TAVI), may influence procedural outcomes.

Diagnosis

Management

  • The cusp-overlap view (COV) technique during TAVI with self-expandable valves is associated with a lower risk of permanent pacemaker implantation (PPI) compared to the coplanar view (CPV) 1.
  • COV is also associated with higher implantation depths during TAVI 1.
  • Special Populations

    Key Recommendations

  • Consider using the cusp-overlap view (COV) technique for transcatheter aortic valve implantation (TAVI) with self-expandable valves to reduce the risk of permanent pacemaker implantation (PPI) 1. (Evidence: Moderate)
  • The cusp-overlap view (COV) technique for TAVI with self-expandable valves may result in deeper valve implantation 1. (Evidence: Moderate)
