Social phobia

Last edited: 4/14/2026

Overview

Social phobia, also known as social anxiety disorder, is characterized by an intense fear of social situations where the individual may be scrutinized, leading to significant distress and impairment in daily functioning 2831.

Diagnosis

  • Persistent fear of social or performance situations where embarrassment might occur
  • Fear disproportionate to actual threat
  • Avoidance of feared situations or enduring with intense anxiety
  • Symptoms last for ≥6 months 2831
  • No specific recommended diagnostic tests; clinical interview and self-report scales are crucial 2831
  • Management

  • First-line treatments:
  • - Cognitive Behavioral Therapy (CBT) 2831 - Selective Serotonin Reuptake Inhibitors (SSRIs): Fluoxetine, Sertraline 31
  • Adjunctive treatments:
  • - Augmentation with benzodiazepines (short-term use due to risk of dependence) 31 - Exposure therapy as part of CBT 28

    Special Populations

  • Pediatrics: Limited specific evidence in abstracts; general principles of CBT and SSRIs apply 33
  • Elderly: Social isolation exacerbated by conditions like sensory loss; interventions focusing on social connectedness may be beneficial 29
  • Comorbidities: Mitral valve prolapse noted in some cases of social phobia; echocardiogram may be considered in selected patients 32
  • Key Recommendations

  • Initiate treatment with Cognitive Behavioral Therapy (CBT) as first-line psychological intervention (Evidence: Strong 2831)
  • Consider Selective Serotonin Reuptake Inhibitors (SSRIs) such as Fluoxetine or Sertraline for pharmacological management (Evidence: Moderate 31)
  • Address social isolation through structured interventions like pen pal programs or virtual service-learning initiatives, particularly in elderly populations (Evidence: Expert opinion 729)
