Liver hematoma

Last edited: 4/14/2026

Overview

Liver hematoma refers to localized bleeding within the liver parenchyma, often secondary to trauma or underlying liver pathology, and can present as a subcapsular collection mimicking other hepatic conditions like metastases. 9

Diagnosis

  • Imaging: Ultrasonography, CT, and MRI are crucial for identifying hematoma characteristics, including echogenicity and size 913.
  • Ultrasonography: Can reveal sonolucent areas indicative of necrosis or hematoma 13.
  • CT: Useful for detailed anatomical assessment and distinguishing from calcified metastases 1115.
  • MRI: Provides additional soft tissue contrast, aiding in differentiation from other lesions 8.
  • LFTs: Elevated liver enzymes may accompany hematoma but are non-specific 14.
  • Management

  • Conservative Management: Often sufficient for asymptomatic subcapsular hematomas, with close monitoring 9.
  • Palliative Radiotherapy: Used for symptomatic relief in cases with massive liver metastases, delivering doses like 17 Gy in multiple fractions 614.
  • Interventional Radiology: Techniques such as radiofrequency ablation, cryotherapy, and laser-induced thermotherapy (LITT) are employed for localized treatment of hepatic lesions, though not specifically for hematomas 78.
  • Special Populations

  • Pregnancy: Subcapsular hematomas can occur, often associated with HELLP syndrome; conservative management is typically recommended with extended postpartum follow-up 9.
  • Key Recommendations

  • Monitor Asymptomatic Hematomas Closely: Regular imaging follow-up is essential to monitor resolution or complications 9 (Evidence: Moderate).
  • Palliative Radiotherapy for Symptomatic Cases: Effective for symptom relief in patients with massive liver metastases, using regimens like 17 Gy in multiple fractions 614 (Evidence: Moderate).
  • Avoid Unnecessary Interventions: Conservative management suffices for many subcapsular hematomas unless there are signs of rupture or significant symptoms 9 (Evidence: Expert opinion).
