Bilateral hilar adenopathy

Last edited: 4/10/2026

Overview

Bilateral hilar adenopathy refers to the enlargement of lymph nodes in both hila of the lungs. This finding can be indicative of various underlying conditions, including infections, inflammatory processes, and malignancies.

Diagnosis

  • Endobronchial ultrasound (EBUS)-guided intranodal forceps biopsy (IFB) in combination with EBUS-guided transbronchial needle aspiration (TBNA) improves the diagnostic yield for intrathoracic adenopathy compared to EBUS-TBNA alone 1.
  • Combined EBUS-IFB and EBUS-TBNA achieved a pooled diagnostic yield of 92% 1.
  • EBUS-TBNA alone yielded a pooled diagnostic yield of 67% 1.
  • Management

  • The addition of EBUS-IFB to EBUS-TBNA improves the overall diagnostic yield of sampling intrathoracic adenopathy 1.
  • Key Recommendations

  • Combined EBUS-IFB and EBUS-TBNA should be considered for improved diagnostic yield in patients with intrathoracic adenopathy 1. (Evidence: Moderate)
  • Combined EBUS-IFB and EBUS-TBNA offers a significantly higher diagnostic yield (92%) compared to EBUS-TBNA alone (67%) 1. (Evidence: Moderate)
