Obstetric nipple infection

Overview

Obstetric nipple infection, often referred to as mastitis or nipple trauma associated with breastfeeding, involves inflammation of the nipple and surrounding breast tissue, commonly seen in lactating women due to cracks, trauma, or bacterial colonization. 1521

Diagnosis

  • Clinical presentation includes painful, red, and swollen nipples with possible discharge.
  • Cultures may be necessary to identify specific pathogens, especially in non-lactating women or cases resistant to initial treatment. 15
  • Mammography or ultrasound can rule out deep infections or abscess formation in complex cases. 21
  • Management

  • Antibiotics: First-line treatment often includes topical or systemic antibiotics, such as topical mupirocin or oral flucloxacillin/clindamycin, depending on local resistance patterns. 15
  • Nipple care: Regular cleaning, air drying, and use of nipple shields or protective creams can prevent further trauma. 15
  • Supportive care: Ensuring proper latch techniques and breastfeeding support can aid recovery. 15
  • Special Populations

  • Lactating women: Focus on maintaining breastfeeding while treating infection to prevent weaning. 15
  • Non-lactating women: Management may involve more aggressive systemic antibiotic therapy due to higher suspicion of underlying conditions. 21
