Parenteral nutrition-related hepatitis

Last edited: 4/16/2026

Overview

Parenteral nutrition-related hepatitis (PN-related hepatitis) refers to liver inflammation and damage observed in patients receiving long-term parenteral nutrition, often due to deficiencies or toxicities associated with the nutritional support.

Diagnosis

  • Elevated liver enzymes (ALT, AST) 68
  • Imaging abnormalities suggestive of liver inflammation 68
  • Exclusion of other causes of liver injury through clinical assessment and laboratory tests 68
  • Liver biopsy may be necessary for definitive diagnosis and grading of severity 68
  • Management

  • Correct deficiencies: Ensure adequate vitamins, particularly fat-soluble vitamins (A, D, E, K) and trace elements 6468
  • Monitor and adjust parenteral nutrition formulation: Avoid excessive amounts of certain lipids and dextrose 68
  • Consider enteral nutrition transition: When feasible, switch to enteral nutrition to reduce PN duration 68
  • Supportive care: Manage complications such as coagulopathy and ascites 68
  • Special Populations

  • Pediatrics: Limited specific data; focus on early detection and correction of deficiencies 2150
  • Elderly: Increased susceptibility to complications; tailored nutritional support and close monitoring recommended 68
  • Comorbidities: Patients with pre-existing liver disease require heightened vigilance and individualized management plans 68
  • Key Recommendations

  • Regular monitoring of liver function tests in PN-dependent patients to early detect PN-related hepatitis (Evidence: Moderate 68)
  • Ensure comprehensive vitamin and trace element supplementation in PN formulations to prevent deficiencies (Evidence: Strong 6468)
  • Transition to enteral nutrition when possible to minimize PN duration and reduce risk of liver complications (Evidence: Moderate 68)
  • Individualize management based on patient-specific factors including age and comorbidities (Evidence: Expert opinion)
