Delirium

Last edited: 4/23/2026

Overview

Delirium is an acute disturbance in mental status characterized by fluctuating inattention and cognitive impairment, often multifactorial in etiology, and frequently observed in critically ill, elderly, and hospitalized patients 16142949.

Diagnosis

  • Key Diagnostic Criteria: Inattention, plus one or more additional cognitive disturbances (e.g., disorganized thinking, hallucinations, delusions) 1649.
  • Recommended Tests: Use validated screening tools such as the Confusion Assessment Method (CAM) or the 4AT in specific settings like ICU and hospice 163969.
  • Grading: Severity can be assessed using tools like the Delirium Rating Scale-Revised (DRS-R) or the Richmond Agitation-Sedation Scale (RASS) 4555.
  • Management

  • First-Line Treatments:
  • - Optimize Underlying Causes: Address infections (e.g., UTIs, pneumonia), metabolic disturbances, and medication side effects 11119374151. - Environmental Modifications: Minimize sensory overload, maintain consistent routines, and ensure adequate lighting and hydration 51548.
  • Adjunctive Treatments:
  • - Non-Pharmacological Interventions: Family involvement, reorientation, and cognitive stimulation 51548. - Pharmacological Interventions: Dexmedetomidine for sedation in ICU patients to potentially reduce delirium incidence 3334. Low-dose ketamine may reduce delirium without affecting opiate consumption 64.

    Special Populations

  • Pediatrics: Targeting key risk factors, such as mechanical ventilation, can reduce delirium risk 1.
  • Elderly: Higher prevalence and complexity due to comorbidities; focus on delirium prevention through risk factor management 16142949.
  • Comorbidities: Diabetes, atherosclerosis, and cerebrovascular risk factors may influence delirium risk, particularly in populations like Native Americans 2.
  • Key Recommendations

  • Target Risk Factors: Implement strategies to mitigate known risk factors (e.g., infections, medication side effects) to reduce delirium incidence (Evidence: Strong 11119374151).
  • Use Validated Screening Tools: Employ tools like CAM, 4AT, and RASS for early detection and monitoring of delirium in appropriate settings (Evidence: Moderate 163969).
  • Optimize Environmental Conditions: Enhance patient environment to minimize delirium triggers through consistent routines and reduced sensory overload (Evidence: Moderate 51548).
  • Consider Dexmedetomidine for ICU Sedation: Use dexmedetomidine in ICU settings to potentially decrease delirium incidence in elderly patients (Evidence: Moderate 3334).
  • Evaluate Medication Use: Scrutinize medication regimens for potential delirium-inducing agents, especially antipsychotics and proton pump inhibitors (Evidence: Moderate 31920).
