Sarcoma of urinary bladder

Last edited: 4/16/2026

Overview

Sarcoma of the urinary bladder is a rare malignancy characterized by malignant transformation of bladder tissue, distinct from more common urothelial carcinomas. It encompasses various histological subtypes, including leiomyosarcomas and rhabdomyosarcomas, often presenting with nonspecific symptoms like hematuria and lower urinary tract symptoms 3352.

Diagnosis

  • Clinical Presentation: Hematuria, lower urinary tract symptoms, and potential bladder obstruction 33.
  • Imaging: CT, MRI, and ultrasound to assess tumor size, local invasion, and regional lymph nodes 33.
  • Biopsy: Definitive diagnosis via transurethral resection or percutaneous biopsy 33.
  • Histopathology: Essential for subtype identification and grading 33.
  • Management

  • Surgical Resection: Primary treatment, often including partial or radical cystectomy depending on tumor stage and extent 33.
  • Adjuvant Therapy: Radiation therapy or chemotherapy may be considered for advanced or high-grade sarcomas 33.
  • Follow-Up: Regular imaging and cystoscopy to monitor for recurrence 33.
  • Special Populations

  • Elderly Patients: Management tailored to comorbidities and functional status; less aggressive approaches may be warranted 54.
  • Comorbidities: Consider impact on treatment tolerance and efficacy; polypharmacy and anticholinergic burden should be minimized 2324.
  • Key Recommendations

  • Surgical Intervention: Primary treatment should involve surgical resection tailored to tumor stage and grade (Evidence: Strong 33).
  • Adjuvant Therapy Consideration: For high-grade or advanced sarcomas, incorporate adjuvant radiation or chemotherapy post-surgery (Evidence: Moderate 33).
  • Comprehensive Follow-Up: Implement rigorous follow-up protocols including imaging and cystoscopy to detect recurrence early (Evidence: Expert opinion 33).
