Chronic low back pain

Last edited: 4/14/2026

Overview

Chronic low back pain (CLBP) is a persistent condition characterized by pain lasting longer than 12 weeks, often associated with structural changes like intervertebral disc degeneration and Modic changes, impacting quality of life and functional capacity 12361935.

Diagnosis

  • Clinical Assessment: History and physical examination to identify pain characteristics, duration, and impact on function 37.
  • Imaging Studies: MRI to assess Modic changes (MC-1 and MC-2), disc degeneration, and vertebral endplate abnormalities 1636.
  • Laboratory Tests: Not typically required unless red flags suggest specific pathology 3.
  • Functional Tests: Assess paraspinal muscle involvement and lumbar spine mechanics 719.
  • Management

  • First-Line Treatments:
  • - Exercise Therapy: Combined with manual therapy for improved pain intensity, disability, and physical function 4. - Physical Therapy: Including aquatic therapy and Pilates for pain reduction and functional improvement 1714. - Pain Neuroscience Education (PNE): To address psychological aspects and improve pain coping mechanisms 9. - Breathing Exercises: Evidence suggests potential benefits, though more research is needed 8.
  • Adjunctive Treatments:
  • - Virtual Reality (VR): Effective for reducing pain and kinesiophobia 515. - Cognitive Behavioral Therapy (CBT): Improves pain, fear avoidance, and self-efficacy 18. - Osteopathic Interventions: Show promise in managing CLBP 21. - Spinal Cord Stimulation (SCS): Effective in refractory cases, with peripheral nerve field stimulation (PNFS) as an add-on therapy 1216. - Tapentadol Prolonged Release: Considered for severe cases with neuropathic components 3034.

    Special Populations

  • Pregnancy: Multiple pregnancies may contribute to chronic lower back pain and lumbar disc degeneration 11.
  • Elderly: Increased prevalence of comorbidities like depression and fibromyalgia affecting CLBP management 3337.
  • Comorbidities: Depression and anxiety significantly impact CLBP management and outcomes 2733.
  • Key Recommendations

  • Combine Exercise Therapy with Manual Therapy for improved functional outcomes and pain reduction 4 (Evidence: Strong).
  • Incorporate Pain Neuroscience Education to address psychological aspects of chronic pain 9 (Evidence: Moderate).
  • Consider Virtual Reality Interventions for patients with kinesiophobia to enhance pain management and functional capacity 515 (Evidence: Moderate).
  • Evaluate and Treat Comorbid Psychological Conditions such as depression and anxiety in CLBP patients 2733 (Evidence: Moderate).
  • Use Spinal Cord Stimulation with PNFS Add-on for patients with refractory chronic low back pain 1216 (Evidence: Moderate).
