Pseudomonas ophthalmia neonatorum

Last edited: 4/14/2026

Overview

Sympathetic ophthalmia is a rare bilateral panuveitis that typically develops following penetrating ocular trauma or certain intraocular procedures, though it can also occur secondary to non-penetrating injuries or treatments like cyclodiode laser 24.

Diagnosis

  • Clinical Presentation: Bilateral granulomatous uveitis following an ocular insult or procedure 24.
  • Histopathology: Presence of epithelioid and giant cells of monocytic origin in uveal infiltrates 89.
  • Immunohistochemistry: Predominant T cells (helper/inducer subset) in uveal infiltrates, with significant monocytic involvement 710.
  • Differential Diagnosis: Exclude other causes of bilateral uveitis such as sarcoidosis, tuberculosis, and Vogt-Koyanagi-Harada syndrome 9.
  • Management

  • Initial Treatment: High-dose corticosteroids to control inflammation 7.
  • Adjunctive Therapy: Immunosuppressive agents such as methotrexate or mycophenolate mofetil if corticosteroids are insufficient 7.
  • Surgical Intervention: Enucleation of the exciting eye may be considered in severe cases to halt disease progression 2.
  • Monitoring: Regular ophthalmic evaluations to monitor response to treatment and prevent complications 7.
  • Special Populations

  • Pediatrics: Case reported of sympathetic ophthalmia in a 10-year-old following cyclodiode laser treatment for rubeotic glaucoma 2.
  • Comorbidities: No specific guidelines provided for comorbidities; management tailored to individual patient needs 7.
  • Key Recommendations

  • Initiate high-dose corticosteroids immediately upon diagnosis to control inflammation (Evidence: Strong 7).
  • Consider immunosuppressive therapy if corticosteroids are not sufficient in controlling symptoms (Evidence: Moderate 7).
  • Evaluate and potentially perform enucleation of the exciting eye in refractory cases to prevent further damage (Evidence: Expert opinion 2).
