Large bowel obstruction

Last edited: 4/14/2026

Overview

Large bowel obstruction (LBO) involves blockage of the distal ileum through the rectum, often due to mechanical causes like adhesions, tumors, or hernias, and can be acute or chronic, requiring prompt diagnosis and management to prevent complications such as bowel ischemia and perforation 2510.

Diagnosis

  • Clinical Presentation: Abdominal pain, distension, vomiting, constipation, and obstipation 210.
  • Physical Examination: Assess for tenderness, palpable mass, and signs of peritonitis 2.
  • Imaging: Abdominal X-rays (including contrast studies), CT scans are crucial for identifying the site and cause of obstruction 210.
  • Laboratory Tests: Elevated white blood cell count may indicate infection or ischemia 2.
  • Endoscopy: Useful in diagnosing specific causes like tumors or strictures 5.
  • Management

  • First-Line Treatments:
  • - Surgical Intervention: Emergency surgery for strangulated obstructions, incarcerated hernias, or non-resolvable obstructions 26. - Colonic Stenting: For malignant obstructions, particularly in the left colon, as a bridge to surgery; efficacy in right-sided obstructions is less established 5.
  • Adjunctive Treatments:
  • - Octreotide: Effective for managing symptoms in inoperable malignant bowel obstruction, reducing nausea, vomiting, and abdominal pain 1113. - Lidocaine Infusion: Considered for severe visceral pain in advanced cancer patients, though primarily for pain management rather than obstruction resolution 3.

    Special Populations

  • Pediatrics: Milk curd syndrome is a rare iatrogenic cause in neonates, requiring prompt recognition and management 1.
  • Elderly: Higher risk of complications; prehabilitation and multimodal treatment approaches may reduce emergency surgery risks 6.
  • Comorbidities: Patients with comorbidities like diabetes may have specific etiologies (e.g., small left colon syndrome) requiring tailored management 7.
  • Key Recommendations

  • Prompt Surgical Intervention for Strangulated Obstructions: Emergency surgery is essential for preventing irreversible ischemia and mortality 2 (Evidence: Strong).
  • Consider Colonic Stenting for Malignant Obstructions: Use colonic stents as a bridge to surgery for left-sided malignant obstructions, with caution in right-sided cases 5 (Evidence: Moderate).
  • Utilize Octreotide for Symptom Management in Inoperable Cases: Administer octreotide for symptom relief in patients with inoperable malignant bowel obstruction 11 (Evidence: Moderate).
  • Prehabilitation for High-Risk Patients: Implement multimodal treatment strategies to reduce the risk of emergency surgery in high-risk elderly patients 6 (Evidence: Moderate).
  • Early Diagnosis and Management in Neonates: Recognize and treat rare causes like milk curd syndrome promptly in neonates 1 (Evidence: Weak).
