Small bowel obstruction

Last edited: 4/14/2026

Overview

Small bowel obstruction (SBO) involves the blockage of the small intestine, often due to adhesions, hernias, or tumors, leading to symptoms like abdominal pain, vomiting, and distension. It is a common surgical emergency requiring prompt diagnosis and management. 16

Diagnosis

  • Clinical Presentation: Abdominal pain, vomiting, distension, and obstipation. 6
  • Imaging: Plain abdominal X-rays often show characteristic findings such as dilated loops of bowel, air-fluid levels, and paucity of gas in the colon. 20
  • Ultrasound: Point-of-care ultrasound can rapidly diagnose SBO by identifying dilated bowel loops and identifying the presence of an obstruction. 3
  • CT Scan: Provides detailed visualization of the bowel and helps differentiate between simple and strangulated obstructions. 6
  • Management

  • Initial Management: Fluid resuscitation, nasogastric tube decompression, and bowel rest. 6
  • Surgical vs. Non-Surgical: Decisions on surgical intervention depend on the clinical scenario; non-surgical approaches like internal splinting with an ileus tube may be considered for selected cases. 13
  • Laparoscopy: Increasingly adopted for adhesional SBO, associated with reduced postoperative length of stay and faster recovery compared to open surgery. 5
  • Postoperative Care: Focus on preventing postoperative ileus (POI) through optimized perioperative management and early mobilization. 2
  • Special Populations

  • Elderly Patients: High morbidity and mortality; comprehensive acute geriatric care is recommended to optimize outcomes. 12
  • Pediatrics: Emergency surgery for conditions like SBNETs may impact long-term outcomes, necessitating careful consideration of timing and approach. 11
  • Comorbidities: Management strategies should account for underlying conditions, such as malignancy, which may influence the choice between surgical and percutaneous interventions. 14
  • Key Recommendations

  • Supervised Resident Involvement: Emergency surgery for acute bowel obstruction can be safely performed under supervision, suggesting feasible training opportunities in specialized settings. (Evidence: Moderate 1)
  • Laparoscopic Approach: Adopt laparoscopic techniques for adhesional SBO to potentially reduce postoperative length of stay and improve recovery times compared to open surgery. (Evidence: Moderate 5)
  • Early Imaging: Utilize imaging, particularly CT scans, early in the diagnostic process to accurately assess the severity and type of obstruction, guiding appropriate management. (Evidence: Moderate 6)
  • Comprehensive Geriatric Care: For elderly patients, integrate comprehensive acute geriatric care to mitigate high morbidity and mortality risks associated with SBO. (Evidence: Expert opinion 12)
  • Consider Non-Surgical Options: Evaluate non-surgical interventions like percutaneous needle decompression for malignant causes, especially when surgical risks are high. (Evidence: Moderate 14)
