Overview
Paroxysmal atrial fibrillation (PAF) is characterized by recurrent episodes of atrial fibrillation lasting less than seven days without a preceding prolonged atrial arrhythmia. These episodes spontaneously revert to sinus rhythm 1.Diagnosis
Electrocardiogram (ECG) showing characteristic AF patterns during symptomatic episodes 1.
Holter monitoring or event recorders for capturing intermittent episodes 2.
Transthoracic echocardiography to rule out structural heart disease 1.
Electrocardiographic criteria for AF duration and recurrence patterns 1.Management
Rate control: Beta-blockers, non-dihydropyridine calcium channel blockers (e.g., verapamil, diltiazem) 1.
Rhythm control: Antiarrhythmic drugs such as class Ic (e.g., flecainide) or class III (e.g., amiodarone) 1.
Anticoagulation: Oral anticoagulants (e.g., warfarin, NOACs) to prevent stroke, especially in patients with CHA2DS2-VASc score ≥1 1.
Electrophysiology study and ablation: Considered for recurrent symptomatic PAF refractory to medical therapy 1.Special Populations
MRI Safety: Insertable cardiac monitors like Reveal XT are generally safe for 3 Tesla MRI in PAF patients, with rare MRI-induced artifacts noted 2.Key Recommendations
Utilize Holter monitoring or insertable loop recorders for diagnosis of intermittent PAF episodes (Evidence: Moderate 2).
Implement rate control strategies with beta-blockers or calcium channel blockers for symptom management (Evidence: Strong 1).
Consider rhythm control with antiarrhythmic drugs or catheter ablation for refractory cases (Evidence: Moderate 1).
Initiate anticoagulation therapy based on stroke risk stratification (Evidence: Strong 1).
Ensure safety protocols for patients with ICMs undergoing MRI procedures (Evidence: Moderate 2).References
