Paroxysmal atrial fibrillation

Overview

Paroxysmal atrial fibrillation (PAF) is characterized by recurrent episodes of atrial fibrillation lasting less than seven days without a preceding prolonged atrial arrhythmia. These episodes spontaneously revert to sinus rhythm 1.

Diagnosis

  • Electrocardiogram (ECG) showing characteristic AF patterns during symptomatic episodes 1.
  • Holter monitoring or event recorders for capturing intermittent episodes 2.
  • Transthoracic echocardiography to rule out structural heart disease 1.
  • Electrocardiographic criteria for AF duration and recurrence patterns 1.
  • Management

  • Electrophysiology study and ablation: Considered for recurrent symptomatic PAF refractory to medical therapy 1.

  • MRI Safety: Insertable cardiac monitors like Reveal XT are generally safe for 3 Tesla MRI in PAF patients, with rare MRI-induced artifacts noted 2.
