Atrial premature complex

Last edited: 4/15/2026

Overview

Atrial premature complexes (APCs) are ectopic beats originating from the atria, preceding the next sinus beat. They can indicate underlying atrial conduction abnormalities and may precede more serious arrhythmias like Mobitz type II second-degree AV block 1.

Diagnosis

  • Electrocardiogram (ECG) Findings: Widely split P waves may indicate atrial conduction delay or block 1.
  • Monitoring for Progression: Evaluate for changes in rhythm, such as progression to Mobitz type II AV block 1.
  • Cardiac Evaluation: Consider consultation with a cardiologist to assess for underlying ischemic or infiltrative processes 1.
  • Management

  • Close Monitoring: Continuous ECG monitoring during perioperative periods to detect arrhythmias 1.
  • Pacemaker Consideration: Implantation of a permanent pacemaker if significant conduction abnormalities or high-risk arrhythmias are identified 1.
  • Anesthetic Technique Adjustment: Modify anesthetic techniques to minimize triggers for serious arrhythmias 1.
  • Special Populations

  • Perioperative Setting: Increased vigilance in patients undergoing surgery, especially those with abnormal P wave morphology on ECG 1.
  • Key Recommendations

  • Evaluate P Wave Morphology: Routinely assess P wave morphology on ECG to identify potential atrial conduction abnormalities 1 (Evidence: Moderate).
  • Cardiologist Consultation: Obtain a cardiologist evaluation for patients with widely split P waves before major surgeries 1 (Evidence: Moderate).
  • Enhanced Monitoring: Implement continuous ECG monitoring during anesthesia in patients with suspected atrial conduction issues 1 (Evidence: Weak).
