Cytomegalovirus infection

Last edited: 4/14/2026

Overview

Human cytomegalovirus (HCMV) infection poses significant risks, particularly in immunocompromised individuals such as transplant recipients, neonates, and those with immunosuppression. Despite the need, no effective mRNA vaccine currently exists, though immunoinformatics approaches are advancing vaccine design 13.

Diagnosis

  • Serological Tests: Commonly used but may lack sensitivity and specificity 3.
  • Molecular Diagnostics: PCR for viral DNA detection in clinical samples 13.
  • Imaging and Histopathology: Useful for organ-specific infections (e.g., CMV colitis, retinitis) 911.
  • Multi-Epitope Antigens: Emerging diagnostic tools for enhanced accuracy 3.
  • Management

  • Antiviral Therapy:
  • - Ganciclovir: First-line treatment for severe infections 810. - Letermovir: Prophylactic use in transplant recipients to prevent CMV disease 52. - Maribavir: Alternative for ganciclovir-resistant cases 4.
  • Adjunctive Treatments:
  • - Immunoglobulins: Used in refractory cases 10. - Leflunomide: Considered in combination therapy for severe infections 10.

    Special Populations

  • Pediatrics: Letermovir effective as prophylaxis in pediatric hematopoietic stem cell transplant recipients 5.
  • Immunocompromised Adults: High risk of severe manifestations including multi-organ failure 810.
  • Elderly: Increased risk due to age-related immune senescence 7.
  • Pregnancy: Congenital CMV infection can occur; monitoring and supportive care are critical 13.
  • Key Recommendations

  • Use Letermovir for Prophylaxis in Transplant Recipients: Effective in preventing CMV disease in both adult and pediatric transplant recipients 5 (Evidence: Strong).
  • Initiate Ganciclovir for Severe CMV Infections: First-line antiviral therapy for managing severe CMV infections, especially in immunocompromised hosts 810 (Evidence: Strong).
  • Regular Ophthalmic Screening for High-Risk Pediatric HSCT Recipients: Essential to detect CMV retinitis early 11 (Evidence: Moderate).
  • Consider Multi-Epitope Antigens for Enhanced Diagnostic Accuracy: Novel diagnostic tools show promise in improving sensitivity and specificity 3 (Evidence: Weak).
  • Monitor for Drug-Drug Interactions with Letermovir: Given its importance in prophylaxis, vigilance for adverse events related to co-administered medications is advised 2 (Evidence: Moderate).
