Infective conjunctivitis

Last edited: 4/14/2026

Overview

Infective conjunctivitis is an inflammation of the conjunctiva primarily caused by bacterial, viral, or parasitic pathogens, leading to symptoms such as redness, discharge, and irritation of the eye. 131120

Diagnosis

  • Clinical Presentation: Redness, purulent discharge, conjunctival swelling, and presence of follicles (in some cases). 131829
  • Physical Examination: Slit-lamp examination to assess conjunctival hyperemia, discharge characteristics, and presence of papillae. 1829
  • Laboratory Tests: Gram stain and culture for bacterial conjunctivitis; PCR for viral conjunctivitis; conjunctival scrapings for parasitic infections. 1930
  • Differential Diagnosis: Rule out keratitis, iritis, and allergic conjunctivitis through appropriate testing. 162327
  • Management

  • Bacterial Conjunctivitis:
  • - First-line: Topical antibiotics such as fortified antibiotic drops (e.g., fortified cefazolin, vancomycin) or fluoroquinolones (e.g., levofloxacin 0.5% q.i.d.). 19
  • Viral Conjunctivitis:
  • - Supportive Care: Artificial tears, cold compresses; avoid topical antivirals unless severe cases (e.g., trifluridine 1% q.i.d.). 323
  • Parasitic Conjunctivitis:
  • - Specific Treatment: Removal of the parasite (e.g., Thelazia worm) and topical antiparasitic agents if necessary. 30
  • Hygiene Measures: Frequent hand washing, avoid sharing towels or eye makeup. 123
  • Special Populations

  • Pregnancy: Use topical antibiotics cautiously; avoid systemic antibiotics unless absolutely necessary. 23
  • Pediatrics: Monitor for signs of systemic infection in neonates; consider bacterial conjunctivitis in newborns as potentially neonatal sepsis. 113
  • Elderly: Increased risk of complications; vigilant monitoring for signs of keratitis or iritis. 1829
  • Comorbidities: Patients with compromised immune systems may require longer treatment courses or systemic antibiotics for severe cases. 19
  • Key Recommendations

  • Diagnose infective conjunctivitis based on clinical presentation and confirm with appropriate laboratory tests (Evidence: Moderate 11929).
  • Initiate targeted antibiotic therapy for bacterial conjunctivitis, guided by culture and sensitivity results (Evidence: Strong 19).
  • Provide supportive care for viral conjunctivitis and consider referral for severe cases (Evidence: Moderate 323).
  • Implement strict hygiene measures to prevent transmission (Evidence: Expert opinion 123).
  • Monitor special populations closely for complications and adjust treatment accordingly (Evidence: Moderate 1131829).
