Urinary tract infection caused by Escherichia coli

Overview

Urinary tract infections (UTIs) are common, particularly in women. Escherichia coli is the most frequent causative pathogen 4. Antimicrobial resistance necessitates exploration of alternative management strategies 4.

Diagnosis

  • Diagnosis of acute uncomplicated UTI is based on the presence of at least one urinary symptom and bacteriuria (>100,000 CFU/ml) 4.
  • Management

  • Antibiotic Therapy:
  • * Fosfomycin, nitrofurantoin, and trimethoprim-sulfamethoxazole (TMP-SMX) are effective for uncomplicated UTIs 1. * Fosfomycin demonstrated the highest efficacy for clinical and microbiological cure in a network meta-analysis 1. * Ciprofloxacin showed the lowest efficacy and highest relapse rate in the same analysis 1.
  • Alternative Therapies:
  • Cranberry products, containing proanthocyanidins (PACs), may prevent UTIs by inhibiting E. coli* adherence to urothelial cells 23. A D-mannose-based dietary supplement (DAPAD complex) showed higher clinical resolution rates compared to placebo for acute uncomplicated E. coli* UTIs 4.
