Acute lung injury

Last edited: 4/23/2026

Overview

Acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) are severe conditions characterized by diffuse alveolar damage, increased permeability, and pulmonary edema, often triggered by sepsis, trauma, or direct lung injury 1.

Diagnosis

  • Clinical criteria include acute onset of respiratory distress, hypoxemia (PaO2/FiO2 < 300 mmHg), and bilateral opacities on imaging 1.
  • Laboratory tests: Elevated white blood cell count, inflammatory markers (e.g., IL-6) 361122.
  • Grading systems: The Berlin Definition categorizes severity based on PaO2/FiO2 ratio, respiratory system compliance, and ventilatory requirements 1.
  • Management

  • Supportive Care: Mechanical ventilation with lung protective strategies (tidal volumes ≤6 mL/kg, plateau pressures <30 cmH2O) 151.
  • Pharmacological Interventions:
  • - Anti-inflammatory Agents: Belnacasan to inhibit pyroptosis 3. - Antioxidants: Edaravone, eugenol, and melatonin to mitigate oxidative stress 111222. - Signaling Pathway Modulators: Curcumin and SO2 targeting PI3K/Akt, p38MAPK, and JAK2/STAT3 pathways 2930.
  • Other Therapies: Electroacupuncture and low-level laser therapy for potential protective effects 1024.
  • Special Populations

  • Pediatrics: Specific considerations for ALI in children include tailored oxygen targets and management strategies 16.
  • Elderly: Higher risk of complications and decisions regarding life support should be individualized 33.
  • Comorbidities: Presence of comorbidities like sepsis or trauma influences both diagnosis and treatment approaches 123.
  • Key Recommendations

  • Implement lung protective ventilation strategies in mechanically ventilated patients to reduce mortality (Evidence: Strong 151).
  • Consider anti-inflammatory and antioxidant therapies such as edaravone and eugenol to mitigate oxidative stress and inflammation (Evidence: Moderate 1112).
  • Tailor oxygen therapy based on patient-specific factors, especially in pediatric populations, to avoid hyperoxia (Evidence: Moderate 16).
  • Individualize decisions regarding life support in elderly patients with ALI due to higher mortality risk (Evidence: Expert opinion 33).
