Injury of lung

Last edited: 4/14/2026

Overview

Lung injury encompasses various traumatic and toxicological causes, including traumatic contusions, sepsis, and toxic exposures such as vaping-associated lung injury (EVALI). These injuries can lead to severe respiratory complications requiring intensive care management.

Diagnosis

  • Clinical Presentation: Symptoms include respiratory distress, hypoxemia, and signs of sepsis or toxic exposure 1710.
  • Imaging: Chest X-rays and CT scans to identify contusions, infiltrates, or signs of acute respiratory distress syndrome (ARDS) 17.
  • Laboratory Tests: Blood cultures, inflammatory markers (CRP, WBC), and specific biomarkers for EVALI (e.g., vitamin E acetate detection) 11011.
  • Histopathology: In cases of electrocution, histopathological examination may reveal lung injury not apparent grossly 7.
  • Management

  • Supportive Care: Mechanical ventilation, oxygen therapy, and hemodynamic stabilization 110.
  • Antibiotics: For suspected or confirmed sepsis 1.
  • Corticosteroids: High-dose methylprednisolone (30 mg/kg i.v. x 3 at 8-hour intervals) for acute traumatic lung injury 20.
  • Toxic Exposure Management: Decontamination procedures post-stabilization, use of antidotes where available 6.
  • Specific EVALI Treatment: Supportive care with extracorporeal membrane oxygenation (ECMO) in severe cases 11.
  • Special Populations

  • Pediatrics: Unintentional exposures more common; lower toxicity but increased risk with age 3.
  • Elderly: Higher susceptibility to complications due to comorbidities; tailored supportive care required 21.
  • Comorbidities: Patients with pre-existing respiratory conditions may have worse outcomes; individualized management plans needed 20.
  • Key Recommendations

  • Early High-Dose Corticosteroid Administration for acute traumatic lung injury to reduce inflammation and improve outcomes (Evidence: Strong 20).
  • Supportive Mechanical Ventilation for patients with severe respiratory failure due to lung injury (Evidence: Moderate 110).
  • Toxic Exposure Decontamination following initial stabilization to prevent further harm (Evidence: Moderate 6).
  • Monitor and Manage Specific Biomarkers in suspected EVALI cases for targeted intervention (Evidence: Moderate 1011).
  • Tailored Care Plans for elderly and pediatric patients considering their unique vulnerabilities (Evidence: Expert opinion 321).
