Pancytopenia caused by colchicine

Last edited: 4/14/2026

Overview

Pancytopenia in colchicine toxicity results from the drug's potent antimitotic effects, disrupting microtubule function and leading to multiorgan dysfunction, including bone marrow suppression 12313.

Diagnosis

  • Clinical Presentation: Progressive gastrointestinal symptoms followed by multiorgan failure, including pancytopenia, shock, and disseminated intravascular coagulation 237.
  • Laboratory Findings: Significant reductions in all blood cell lines (pancytopenia), often with absolute neutropenia 2313.
  • Supportive Tests: Blood concentration analysis may aid in diagnosis but lacks specificity 1.
  • Management

  • Supportive Care: Fluid resuscitation, monitoring for electrolyte imbalances, and management of organ failure 2313.
  • Granulocyte Colony-Stimulating Factor (G-CSF): Administered to manage severe neutropenia and pancytopenia 13.
  • Specific Therapy: Anticolchicine monoclonal antibodies are emerging but not widely available; current treatment remains largely supportive 1312.
  • Special Populations

  • Elderly: Higher risk of severe complications due to cumulative effects 14.
  • Pediatrics: Case reports indicate severe toxicity even at lower doses relative to body weight 9.
  • Key Recommendations

  • Initiate Immediate Supportive Measures for fluid balance, electrolyte correction, and organ support in suspected colchicine toxicity (Evidence: Moderate 23).
  • Monitor Hematologic Parameters Closely for early detection of pancytopenia and initiate G-CSF if neutropenia is severe (Evidence: Weak 13).
  • Consider Early Use of G-CSF in cases of significant bone marrow suppression to mitigate complications (Evidence: Weak 13).
  • Avoid Specific Antidotes due to lack of availability; focus on supportive care until new therapies like anticolchicine Fab fragments become standard (Evidence: Expert opinion 12).
