Waldenstrom's macroglobulinemia

Overview

Waldenstrom's macroglobulinemia (WM) is a rare type of non-Hodgkin lymphoma characterized by lymphoplasmacytic infiltration, monoclonal IgM paraproteinemia, and often the MYD88 L265P mutation 12.

Diagnosis

  • Presence of lymphoplasmacytic cells in bone marrow or other organs
  • Monoclonal IgM paraproteinemia in serum
  • MYD88 L265P mutation and CXCR4 mutations influence clinical course and prognosis 23
  • Diagnostic workup includes bone marrow biopsy, serum protein electrophoresis, and genetic testing 3
  • Management

  • Asymptomatic patients do not require immediate treatment 1
  • Symptomatic patients treated with alkylating agents, proteasome inhibitors (e.g., bortezomib), anti-CD20 monoclonal antibodies (e.g., rituximab), and BTK inhibitors (e.g., ibrutinib) 1
