Ulnar neuropathy

Last edited: 4/14/2026

Overview

Ulnar neuropathy at the elbow (UNE) is a common entrapment neuropathy characterized by symptoms such as diminished sensation in the fourth and fifth digits and pain in the medial elbow region, often resulting from mechanical compression or ischemia of the ulnar nerve.

Diagnosis

  • Clinical Symptoms: Diminished sensation in the ulnar nerve distribution (fourth and fifth digits) and medial elbow pain 3.
  • Recommended Tests: Neuromuscular ultrasound to measure ulnar nerve cross-sectional area (CSA) or diameter for accurate diagnosis and localization 1.
  • Electrophysiological Studies: Nerve conduction studies may be used to confirm diagnosis 4.
  • Pressure Assessment: Consider prolonged elbow flexion pressure as a risk factor 24.
  • Management

  • Conservative Treatment: Rest, avoidance of elbow flexion, and ergonomic adjustments 24.
  • Physical Therapy: May include exercises to relieve nerve compression 3.
  • Surgical Intervention: Indicated for persistent symptoms unresponsive to conservative measures; involves exploration and decompression of the ulnar nerve 3.
  • Specific Interventions: No specific drug classes or doses are highlighted in the abstracts.
  • Special Populations

  • Occupational Risk: Prolonged laptop use or computer work with elbow pressure may increase risk 24.
  • Sports Activities: Nordic skiing and prolonged bicycle riding can precipitate ulnar neuropathy 57.
  • Medical Procedures: Cryotherapy near the ulnar nerve at the elbow poses a risk 6.
  • Key Recommendations

  • Offer neuromuscular ultrasound measurement of ulnar nerve CSA or diameter to confirm UNE diagnosis and localize compression in symptomatic patients (Evidence: Moderate 1).
  • Consider ergonomic modifications and avoidance of prolonged elbow flexion in occupational settings to prevent UNE (Evidence: Moderate 24).
  • Explore surgical decompression for patients with persistent symptoms despite conservative management (Evidence: Expert opinion 3).
