Open fracture of hip

Last edited: 4/23/2026

Overview

Open fractures of the hip are severe injuries characterized by bone disruption and potential contamination, leading to significant morbidity and mortality, particularly in elderly patients. These injuries require urgent surgical intervention and comprehensive multidisciplinary care to optimize outcomes.

Diagnosis

  • Clinical Presentation: Severe pain, inability to bear weight, deformity, and signs of shock or neurovascular compromise.
  • Imaging:
  • - X-rays: Initial imaging to confirm fracture type and displacement. - CT/MRI: For detailed assessment of soft tissue injuries, joint involvement, and planning surgical approaches.
  • Laboratory Tests:
  • - Complete blood count (CBC) to assess for anemia or coagulopathy. - Electrolytes, renal function, and liver function tests.
  • Venous Thromboembolism (VTE) Risk Assessment: Evaluate for risk factors including immobility, age, and comorbidities 71749.
  • Management

  • Surgical Intervention:
  • - Timing: Early surgical fixation within 24-48 hours to reduce complications 4457. - Technique: Depends on fracture type (osteosynthesis, hemiarthroplasty, total hip arthroplasty) 17.
  • Pain Management:
  • - Multimodal analgesia including NSAIDs, opioids, and regional anesthesia (e.g., LOH block) 355.
  • Infection Prevention:
  • - Prophylactic antibiotics tailored to local resistance patterns 65.
  • Thromboprophylaxis:
  • - Direct oral anticoagulants (DOACs) or low molecular weight heparin (LMWH) for VTE prophylaxis 64464.
  • Anemia Management:
  • - Address moderate to severe anemia preoperatively to reduce complications 27.

    Special Populations

  • Elderly Patients:
  • - Higher risk of complications including delirium, depression, and increased mortality 2549. - Consider frailty assessment and multidisciplinary orthogeriatric care 102257.
  • Comorbidities:
  • - Presence of comorbidities like heart failure, respiratory issues, and polypharmacy impacts outcomes 225268. - Evaluate and deprescribe unnecessary medications to improve safety 1258.

    Key Recommendations

  • Early Surgical Fixation: Perform surgery within 24-48 hours to minimize complications and improve outcomes (Evidence: Strong 4457).
  • Multimodal Analgesia: Utilize a combination of pharmacological and regional anesthesia techniques for effective pain control (Evidence: Moderate 355).
  • Thromboprophylaxis: Implement DOACs or LMWH for VTE prophylaxis to reduce the risk of thromboembolic events (Evidence: Strong 664).
  • Comprehensive Geriatric Assessment: Incorporate frailty evaluation and orthogeriatric co-management to address multimorbidity and improve patient outcomes (Evidence: Moderate 102257).
  • Optimize Medication Review: Conduct medication reviews to deprescribe unnecessary medications, particularly in polypharmacy scenarios (Evidence: Moderate 1258).
