Pneumothorax

Last edited: 4/13/2026

Overview

Pneumothorax is the presence of air in the pleural space, leading to lung collapse and potential respiratory compromise, which can occur spontaneously or secondary to trauma, surgery, or underlying lung conditions. 3

Diagnosis

  • Clinical Presentation: Sudden chest pain, shortness of breath, and decreased breath sounds on affected side. 123
  • Diagnostic Imaging: Chest X-ray is the initial imaging modality, with thoracic ultrasound (TUS) emerging as a non-inferior alternative for detection. 49
  • Ultrasound Findings: Absence of pleural sliding and presence of lung point or comet-tail artifacts indicative of pneumothorax. 418
  • CT Scan: Useful for complex cases, quantifying pneumothorax size, and identifying underlying lung pathology. 11
  • Management

  • Small Primary Spontaneous Pneumothorax (PSP): Simple aspiration as initial treatment; success rates vary but can be effective. 2729
  • Large or Recurrent PSP: Chest tube insertion, often with one-way valve systems for potential outpatient management. 1037
  • Tension Pneumothorax: Immediate needle decompression followed by chest tube insertion; simulation-based training recommended for proficiency. 16
  • Non-Invasive Management: Considered in very elderly patients with small pneumothoraces, with adequate monitoring. 20
  • Special Populations

  • Pregnancy: Management requires careful consideration to avoid radiation exposure; thoracostomy tube placement may be necessary with close monitoring. 45
  • Elderly: Non-interventional management (NIM) can be successful for small pneumothoraces with appropriate follow-up. 20
  • Comorbidities: Patients with underlying lung diseases may require more aggressive interventions; individualized treatment plans are essential. 19
  • Key Recommendations

  • Use Thoracic Ultrasound for Initial Pneumothorax Detection: TUS is non-inferior to chest radiography and can reduce unnecessary imaging. (Evidence: Moderate 418)
  • Consider Simple Aspiration for Small Primary Spontaneous Pneumothorax: Effective initial treatment with variable success rates. (Evidence: Moderate 2729)
  • Implement Simulation Training for Chest Tube Insertion: Enhances procedural skills and reduces complications. (Evidence: Expert opinion 6)
  • Adopt Non-Invasive Management Strategies for Appropriate Elderly Patients: NIM can be successful for small pneumothoraces with close monitoring. (Evidence: Weak 20)
  • Immediate Needle Decompression for Tension Pneumothorax: Essential life-saving intervention followed by chest tube placement. (Evidence: Strong 1)
