Addison's disease

Overview

Addison's disease, also known as primary adrenal insufficiency, is a rare autoimmune disorder characterized by inadequate production of cortisol and often aldosterone by the adrenal glands, leading to adrenal insufficiency 8.

Diagnosis

  • Clinical Features: Hyperpigmentation, fatigue, weight loss, hypotension, hyperpigmented scars, and salt craving 17.
  • Laboratory Tests: Hyponatremia, hyperkalemia, elevated ACTH levels, and low cortisol levels (especially on stimulation tests like ACTH stimulation test) 1418.
  • Imaging: Abdominal imaging may show atrophic or enlarged adrenal glands 30.
  • Autoantibodies: Presence of adrenal cortex antibodies can support the diagnosis 835.
  • Management

  • Glucocorticoids: Replacement therapy with hydrocortisone or equivalent (dose adjusted based on individual needs) 13.
  • Mineralocorticoids: Fludrocortisone for aldosterone deficiency 13.
  • Sodium Supplementation: During acute illness or "sick day rules," increased sodium intake and possibly additional hydrocortisone 13.
  • Vitamin D: High-dose vitamin D therapy may modulate immune response and potentially benefit immune dysregulation 67.
  • Special Populations

  • Pregnancy: Adequate glucocorticoid and mineralocorticoid replacement is crucial; monitor closely for adrenal crisis 2532.
  • Pediatrics: X-linked congenital forms exist, requiring early diagnosis and management 34.
  • Comorbidities: Concurrent conditions like thyrotoxicosis can complicate management; treat underlying conditions to prevent adrenal crises 23.
  • Key Recommendations

  • Initiate glucocorticoid replacement therapy with hydrocortisone or equivalent to maintain normal cortisol levels (Evidence: Strong 13).
  • Monitor and adjust mineralocorticoid therapy with fludrocortisone to manage electrolyte imbalances, particularly hyponatremia and hyperkalemia (Evidence: Strong 13).
