Nonulcer dyspepsia

Overview

Nonulcer dyspepsia (NUD) refers to chronic or recurrent upper abdominal discomfort without evidence of organic gastrointestinal disease on endoscopy or laboratory tests 13.

Diagnosis

  • Unexplained dyspepsia symptoms without endoscopic abnormalities or abnormal blood tests 13.
  • Consider testing for Helicobacter pylori infection in patients with NUD 1.
  • Routine endoscopic gastric biopsies for H. pylori detection in NUD patients are not recommended 3.
  • Management

  • Test for and treat H. pylori infection if present, particularly in patients with unexplained dyspepsia 1.
  • Antibiotic therapy may benefit approximately one-fourth of patients with H. pylori-positive NUD 1.
  • No strong evidence supports routine H. pylori eradication for all NUD patients 23.
