Retinoblastoma

Last edited: 4/14/2026

Overview

Retinoblastoma is the most common intraocular malignancy in children, arising from mutations in the RB1 gene affecting retinal cell proliferation 15.

Diagnosis

  • Clinical Presentation: Leukocoria, strabismus, and visual impairment 312.
  • Diagnostic Tests:
  • - Imaging: Ultrasound, MRI, and CT scans for tumor extension and staging 1. - Fine Needle Aspiration Biopsy (FNAB): Useful for confirming diagnosis in selected cases 18. - Genetic Testing: Essential for identifying hereditary forms 21.
  • Referral Accuracy: Referring physicians often misdiagnose retinoblastoma; tertiary centers are crucial for accurate diagnosis 12.
  • Management

  • First-Line Treatments:
  • - Intra-arterial Chemotherapy (IAC): Primary treatment option using melphalan, carboplatin, and topotecan 2. - Enucleation: For advanced or unresectable tumors 113. - Light Coagulation and Radiotherapy: Conservative treatments for smaller tumors 3637.
  • Adjunctive Treatments:
  • - Chemotherapy: Systemic agents like vincristine, carboplatin, and etoposide in combination with IAC 2. - Interstitial Brachytherapy: Requires sedation; S(+)-ketamine for prolonged procedures 15.

    Special Populations

  • Pediatrics: Early diagnosis and multidisciplinary care are critical 312.
  • Developing Countries: Delayed presentation and advanced disease stages are common due to lack of awareness 20.
  • Key Recommendations

  • Immediate Referral to Tertiary Centers: Essential for accurate diagnosis and management of retinoblastoma 12 (Evidence: Strong).
  • Utilize Imaging Techniques: Regular use of ultrasound, MRI, and CT for staging and monitoring tumor progression 1 (Evidence: Strong).
  • Consider Genetic Testing: Crucial for identifying hereditary forms and guiding family screening 21 (Evidence: Moderate).
  • Intra-arterial Chemotherapy as Primary Treatment: Recommended for localized delivery of chemotherapeutic agents 2 (Evidence: Moderate).
  • Conservative Treatments for Early-Stage Tumors: Light coagulation and radiotherapy can be effective in preserving vision 3637 (Evidence: Moderate).
