Goiter

Overview

Goiter refers to the abnormal enlargement of the thyroid gland, which can be classified as substernal (involving the mediastinum) or retrosternal (extending behind the sternum). It often results from iodine deficiency but can also be associated with other etiologies such as autoimmune disorders or multinodular hyperplasia 118.

Diagnosis

  • Clinical Examination: Palpation of the neck to assess size and location of the goiter 18.
  • Imaging: Ultrasound, CT, or MRI to evaluate the extent and intrathoracic involvement 1719.
  • Thyroid Function Tests: To assess for hypothyroidism or hyperthyroidism 211.

  • Medical Management:
  • - Thyroid Hormone Therapy: Not routinely recommended for simple goiters unless symptomatic or growing 2.

  • Pregnancy: Airway management with awake fiberoptic intubation followed by combined caesarean section and thyroidectomy if airway obstruction occurs 16.
