Pancreatic insufficiency

Last edited: 4/14/2026

Overview

Pancreatic insufficiency refers to inadequate exocrine pancreatic function, leading to maldigestion and malabsorption of nutrients, often characterized by steatorrhea and deficiencies in fat-soluble vitamins 8.

Diagnosis

  • Clinical Presentation: Steatorrhea, abdominal pain, weight loss, and nutrient deficiencies 8.
  • Laboratory Tests: Fecal elastase-1 levels, fecal fat test, and serum levels of vitamins A, D, E, and K 2.
  • Imaging and Endoscopy: ERCP with pancreatic function tests, EUS for structural assessment 5.
  • Management

  • Pancreatic Enzyme Replacement Therapy (PERT):
  • - Pancrelipase: Typically dosed at 1000-2000 units of lipase per meal (adjust based on response and fat content of meals) 2.
  • Dietary Modifications: High-protein, low-fat diet with supplementation of fat-soluble vitamins 2.
  • Monitoring: Regular follow-up to assess nutritional status and adjust PERT as needed 2.
  • Special Populations

  • Radiation-Induced Insufficiency: Consider both pancreatic and enteric causes of steatorrhea post-irradiation 8.
  • Key Recommendations

  • Initiate Pancrelipase Therapy for confirmed pancreatic insufficiency to improve digestion and absorption (Evidence: Moderate 2).
  • Regular Follow-Up is essential to monitor response to therapy and nutritional status (Evidence: Expert opinion 2).
  • Consider Dual Etiology in patients with steatorrhea following abdominal irradiation, evaluating both pancreatic function and enteric causes (Evidence: Weak 8).
