Adrenal cortical hypofunction

Last edited: 4/14/2026

Overview

Adrenal cortical hypofunction, often associated with conditions like Addison's disease, involves insufficient production of glucocorticoids and often mineralocorticoids, leading to symptoms such as fatigue, hypotension, and hyperpigmentation. 39

Diagnosis

  • Clinical symptoms: Fatigue, weakness, hypotension, hyperpigmentation, salt craving, and gastrointestinal disturbances. 39
  • Laboratory tests: Elevated ACTH levels, low serum cortisol, and in cases of primary adrenal insufficiency, elevated renin and aldosterone levels. 39
  • Confirmatory tests: ACTH stimulation test showing inadequate cortisol response. 39
  • Imaging: Not typically required unless considering adrenal pathology; MRI or CT may be used to rule out structural causes. 37
  • Management

  • First-line treatment: Glucocorticoid replacement (e.g., hydrocortisone, prednisolone) tailored to individual needs. 39
  • Mineralocorticoid replacement: Fludrocortisone for patients with mineralocorticoid deficiency. 39
  • Dose adjustment: Regular monitoring and adjustment based on ACTH stimulation test results and clinical symptoms. 39
  • Hydration and salt intake: Advising increased salt intake and monitoring for dehydration, especially during stress or illness. 39
  • Special Populations

  • Pediatrics: Abnormal linear growth can occur due to adrenal hormone deficiencies; growth monitoring and appropriate hormone replacement are crucial. 14
  • Elderly: Increased susceptibility to complications like hypotension and electrolyte imbalances; careful management and monitoring are essential. 39
  • Comorbidities: Patients with coexisting autoimmune conditions may require tailored immunosuppressive strategies alongside adrenal replacement therapy. 39
  • Key Recommendations

  • Initiate glucocorticoid replacement therapy based on clinical presentation and laboratory findings to normalize cortisol levels. (Evidence: Strong 39)
  • Add mineralocorticoid replacement (fludrocortisone) in patients with confirmed mineralocorticoid deficiency to manage electrolyte balance. (Evidence: Strong 39)
  • Regularly monitor patients with adrenal cortical hypofunction for signs of inadequate replacement and adjust doses accordingly, especially during periods of stress. (Evidence: Moderate 39)
