Subacute thyroiditis

Last edited: 4/14/2026

Overview

Subacute thyroiditis, also known as de Quervain's thyroiditis, is an inflammatory condition of the thyroid gland often triggered by viral infections, characterized by transient thyrotoxicosis followed by hypothyroidism. 16

Diagnosis

  • Clinical Symptoms: Fever, neck pain, tenderness, and transient thyrotoxicosis followed by hypothyroidism. 110
  • Laboratory Tests: Elevated ESR and CRP, low radioactive iodine uptake, presence of thyroglobulin antibodies. 15
  • Imaging: Ultrasound showing a "washed-out" appearance in the active phase; helpful for unilateral cases and differentiating cysts from hemorrhagic degeneration. 9
  • Differential Diagnosis: Includes giant cell arteritis, vocal cord paralysis, and infections like Chlamydia psittaci. 3678
  • Management

  • First-Line Treatment: Glucocorticoids (e.g., prednisone) to reduce inflammation; dose and duration vary but typically tapered over weeks. 14
  • Adjunctive Treatments: Symptomatic management including analgesics for pain, thyroid hormone replacement if hypothyroidism develops. 110
  • Infection Management: Aggressive antibiotic therapy if infective complications like thyroid abscess occur, especially in patients on steroids. 4
  • Special Populations

  • Comorbidities: Patients with type 2 diabetes mellitus may require careful monitoring and management of blood glucose levels alongside thyroid treatment. 1
  • Vocal Cord Paralysis: Rare complication; may persist post-thyroiditis recovery; requires ENT evaluation and management. 78
  • Key Recommendations

  • Initiate glucocorticoid therapy for symptomatic subacute thyroiditis to manage inflammation and symptoms effectively. (Evidence: Moderate 14)
  • Monitor closely for complications such as vocal cord paralysis and thyroid abscess, especially in patients on steroid therapy. (Evidence: Weak 74)
  • Consider differential diagnoses including giant cell arteritis and infections when evaluating atypical presentations. (Evidence: Expert opinion 36)
