Asystole

Last edited: 4/23/2026

Overview

Asystole is a cardiac rhythm characterized by the absence of electrical activity in the heart, leading to cessation of mechanical cardiac output and potentially sudden cardiac collapse 1.

Diagnosis

  • Episodes of prolonged (>20 seconds) absence of QRS complexes on ECG 1.
  • History of syncope or near-syncope often reported 1.
  • Occasional patients may experience dream-like states without overt symptoms 1.
  • No specific laboratory tests; focus on detailed clinical history and ECG monitoring 1.
  • Management

  • Immediate resuscitation measures including airway management, ventilation, and cardiopulmonary resuscitation (CPR) 1.
  • Consider underlying causes such as electrolyte imbalances, hypoxia, or drug toxicity, and correct as necessary 1.
  • Defibrillation is not effective for asystole; avoid inappropriate use 1.
  • Temporary cardiac pacing may be considered in refractory cases 1.
  • Special Populations

  • No specific data provided for pregnancy, pediatrics, elderly, or comorbidities in the given abstracts 1.
  • Key Recommendations

  • Initiate immediate resuscitation efforts including CPR and airway support for suspected asystole episodes (Evidence: Expert opinion) 1.
  • Thoroughly investigate and correct reversible causes such as electrolyte disturbances and hypoxia (Evidence: Expert opinion) 1.
  • Avoid defibrillation in cases of asystole as it is ineffective (Evidence: Expert opinion) 1.
