Ochronotic arthritis

Last edited: 4/14/2026

Overview

Ochronotic arthritis, associated with alkaptonuria, results from the accumulation of homogentisic acid leading to degenerative joint disease, often affecting large cartilaginous surfaces like knees, spine, and shoulders, and can involve cardiovascular complications such as aortic stenosis 13568.

Diagnosis

  • Clinical Features: Dark urine, ochronotic pigmentation (blue-black discoloration of skin, sclera, ear cartilage) 132.
  • Laboratory Tests: Elevated levels of homogentisic acid in urine 1.
  • Imaging: MRI can reveal characteristic changes in affected joints, particularly in the spine 4.
  • Cardiac Evaluation: Echocardiography or cardiac MRI for aortic valve calcification and stenosis 56.
  • Management

  • Symptomatic Relief: Analgesics and nonsteroidal anti-inflammatory drugs (NSAIDs) for pain management 13.
  • Joint Interventions: Total joint replacement for severe joint damage, particularly hips and knees 7.
  • Cardiac Complications: Surgical intervention such as aortic valve replacement for severe aortic stenosis 56.
  • Supportive Care: Physical therapy to maintain joint function and mobility 13.
  • Special Populations

  • Elderly: Increased prevalence of severe joint involvement and cardiovascular complications 125.
  • Comorbidities: Concurrent conditions like gouty arthritis and atherosclerosis are noted 12.
  • Familial Cases: Inheritance patterns suggest genetic counseling may be beneficial for affected families 5.
  • Key Recommendations

  • Diagnose ochronotic arthritis through clinical features and elevated homogentisic acid levels in urine (Evidence: Moderate 1).
  • Consider MRI for detailed assessment of joint damage, especially in the spine (Evidence: Weak 4).
  • Evaluate cardiovascular status with echocardiography to detect aortic valve calcification (Evidence: Weak 56).
  • Perform joint replacement surgery for end-stage joint disease (Evidence: Expert opinion 7).
  • Manage aortic stenosis with urgent surgical intervention if indicated (Evidence: Expert opinion 6).
