Chronic respiratory failure

Last edited: 4/14/2026

Overview

Chronic respiratory failure is a condition characterized by inadequate gas exchange leading to systemic hypoxemia and/or hypercapnia, often requiring mechanical ventilation support, either invasive or non-invasive, and potentially managed at home with telemedicine support. 16

Diagnosis

  • Clinical Presentation: Symptoms include dyspnea, fatigue, and signs of respiratory distress.
  • Pulmonary Function Tests: Spirometry showing restrictive or obstructive patterns.
  • Blood Gas Analysis: Evidence of hypoxemia and/or hypercapnia.
  • Imaging: Chest X-rays or CT scans to assess lung structure and rule out other causes.
  • Sleep Studies: Useful in identifying sleep-disordered breathing contributing to respiratory failure. 3
  • Management

  • Non-Invasive Ventilation (NIV): First-line for chronic respiratory failure, particularly in elderly patients 3.
  • Home Mechanical Ventilation (HMV): Initiation with telemedicine support can be conditionally recommended 1.
  • Palliative Care: Integration of hospice palliative care can influence medical service utilization positively in prolonged mechanical ventilation cases 2.
  • Tele-Monitoring: Utilization of remote health monitoring systems for safety and feasibility in ventilator-dependent patients 6.
  • Special Populations

  • Elderly: NIV can be effectively used in older adults; age should not be a contraindication 3.
  • Pediatrics: Infants and children with chronic respiratory failure can be successfully managed with home mechanical ventilation 9.
  • Comorbidities: Specific considerations for patients requiring urgent surgical interventions under regional anesthesia with NIV support 8.
  • Key Recommendations

  • Consider Telemedicine for HMV Initiation: Conditional recommendation for initiating home mechanical ventilation with telemedicine support in patients with neuromuscular disease 1 (Evidence: Moderate).
  • Integrate Palliative Care: Utilize hospice palliative care to potentially reduce ICU admissions and emergency interventions in prolonged mechanical ventilation cases 2 (Evidence: Moderate).
  • Use Non-Invasive Ventilation in Elderly: Employ non-invasive ventilation as a viable treatment option in elderly patients with chronic respiratory failure 3 (Evidence: Expert opinion).
