Basal cell carcinoma with adnexal differentiation

Last edited: 4/10/2026

Overview

Basal cell carcinoma (BCC) with adnexal differentiation refers to a rare subset of BCCs exhibiting features of hair follicle or sebaceous gland differentiation. These tumors are uncommon and often present on the head and neck 1.

Diagnosis

  • Histopathological examination is required for diagnosis, identifying features of sebaceous differentiation within a BCC 1.
  • Muir-Torre syndrome (MTS) should be considered and ruled out if suspected 1.
  • Management

  • Complete surgical excision with microscopically controlled margins is the primary treatment modality 1.
  • Adjuvant or therapeutic radiotherapy may be considered 1.
  • For advanced, inoperable, or metastatic disease, management should be individualized by an interdisciplinary tumor board, potentially including local procedures, chemotherapy, or immunotherapy 1.
  • Close follow-up is recommended 1.
  • Key Recommendations

  • Complete micrographically controlled surgery (MCS) of the primary tumor is the therapy of first choice for sebaceous gland carcinomas with adnexal differentiation 1. (Evidence: Expert opinion)
  • Muir-Torre syndrome should always be ruled out if suspected in cases of sebaceous gland carcinoma 1. (Evidence: Expert opinion)
  • Adjuvant or therapeutic radiotherapy may be considered for sebaceous gland carcinomas 1. (Evidence: Expert opinion)
