Fibroblastic rheumatism

Last edited: 4/13/2026

Overview

Fibroblastic rheumatism, also known as proliferative synovitis, is a rare inflammatory condition characterized by the proliferation of fibrous tissue in the synovium, leading to joint stiffness, swelling, and functional impairment. It often affects the hands and feet and can be associated with underlying systemic diseases or as a primary entity 17.

Diagnosis

  • Clinical presentation includes joint swelling, pain, and stiffness, particularly in the hands and feet 17.
  • Imaging studies such as X-rays may show soft tissue swelling but are often non-specific; MRI can reveal synovial proliferation and thickening 17.
  • Histopathological examination of synovial tissue biopsy is crucial for definitive diagnosis, showing fibrous tissue proliferation 17.
  • Laboratory tests typically show nonspecific inflammatory markers (elevated ESR, CRP) 17.
  • Management

  • First-line treatments: Nonsteroidal anti-inflammatory drugs (NSAIDs) to manage pain and inflammation 17.
  • Second-line treatments: Corticosteroids, either intra-articular or systemic, for more severe cases 17.
  • Adjunctive therapies: Disease-modifying antirheumatic drugs (DMARDs) such as methotrexate may be considered in refractory cases, though evidence is limited 17.
  • Physical therapy: Recommended to maintain joint mobility and function 17.
  • Special Populations

  • Pediatrics: Limited data; management focuses on symptomatic relief and physical therapy 19.
  • Elderly: Care must consider comorbidities; cautious use of systemic corticosteroids due to potential side effects 24.
  • Comorbidities: Management should address underlying systemic conditions if present, tailoring therapy accordingly 17.
  • Key Recommendations

  • Perform synovial biopsy for definitive diagnosis of fibroblastic rheumatism (Evidence: Moderate 17).
  • Initiate treatment with NSAIDs for symptomatic relief and to manage inflammation (Evidence: Expert opinion 17).
  • Consider intra-articular or systemic corticosteroids for refractory cases (Evidence: Moderate 17).
  • Incorporate physical therapy to preserve joint function and mobility (Evidence: Expert opinion 17).
  • Tailor treatment plans considering potential comorbidities, especially in elderly patients (Evidence: Expert opinion 24).
