Chronic autoimmune urticaria

Last edited: 4/14/2026

Overview

Chronic autoimmune urticaria, specifically chronic spontaneous urticaria (CSU), is characterized by recurrent hives lasting more than six weeks, often significantly impacting patients' quality of life 14.

Diagnosis

  • Key Criteria: Recurrent wheals and/or angioedema persisting for >6 weeks 4.
  • Recommended Tests: Exclusion of physical triggers, thyroid function tests to rule out autoimmune associations 411.
  • Grading: Severity can be assessed using urticaria activity scores or visual analog scales 7.
  • Management

  • First-Line Treatments: Second-generation H1 antihistamines (e.g., cetirizine, levocetirizine) up to 4x daily dose 515.
  • Adjunctive Treatments:
  • - Oral Corticosteroids: Short-term use for severe flares 4. - Leukotriene Receptor Antagonists: Montelukast as add-on therapy 4. - Omalizumab: For refractory cases, typically administered subcutaneously 813.
  • Vitamin D Supplementation: Considered based on low levels, though not standard 510.
  • Special Populations

  • Pediatrics: Helicobacter pylori should be considered in cases unresponsive to conventional treatment 11.
  • Comorbidities: Metabolic syndrome may be associated with CSU, warranting screening 6.
  • Key Recommendations

  • Initiate second-generation H1 antihistamines as first-line therapy (Evidence: Strong 515).
  • Consider omalizumab for patients refractory to antihistamines and corticosteroids (Evidence: Moderate 813).
  • Evaluate and manage comorbidities such as thyroid disorders and metabolic syndrome (Evidence: Moderate 46).
  • Assess vitamin D levels and consider supplementation in deficient patients (Evidence: Weak 510).
  • Screen for Helicobacter pylori in pediatric cases with chronic urticaria unresponsive to treatment (Evidence: Expert opinion 11).
