Factitious skin disease

Last edited: 4/23/2026

Overview

Factitious skin disease, also known as factitious disorder imposed on self (FDIS), involves patients intentionally producing or feigning physical symptoms affecting their skin, often seeking medical attention and sympathy. 25

Diagnosis

  • Clinical Presentation: Patients present with inconsistent or contradictory symptoms, signs of self-infliction, or fabricated histories. 25
  • Psychological Evaluation: Essential to rule out malingering or other psychiatric conditions; often requires a multidisciplinary approach. 25
  • Differential Diagnosis: Exclude other dermatological conditions and psychiatric disorders like somatization disorder or Munchausen syndrome by proxy. 25
  • Collaborative Assessment: Involves dermatologists, psychiatrists, and sometimes psychologists to confirm the diagnosis through thorough history-taking and observation. 25
  • Management

  • Psychiatric Intervention: Primary treatment involves psychotherapy, particularly cognitive-behavioral therapy (CBT) and psychodynamic therapy. 25
  • Medication: Antidepressants or antipsychotics may be used to manage comorbid psychiatric conditions, though specific drug classes and doses are not detailed in the abstracts. 25
  • Supportive Care: Family involvement and support groups can be beneficial, though specific protocols are not outlined. 25
  • Monitoring and Follow-Up: Regular psychiatric follow-ups are crucial to monitor progress and adjust treatment plans as necessary. 25
  • Special Populations

  • Pediatrics: Diagnosis and management in children require heightened vigilance and parental involvement, though specific guidelines are not provided in the abstracts. 25
  • Elderly: Older adults may present unique challenges due to comorbid conditions and potential polypharmacy, though detailed recommendations are lacking. 25
  • Key Recommendations

  • Multidisciplinary Approach: Engage dermatologists and psychiatrists collaboratively for accurate diagnosis and comprehensive treatment planning. (Evidence: Moderate 25)
  • Psychotherapy as First-Line: Prioritize psychotherapy, particularly CBT, for managing factitious skin disease symptoms. (Evidence: Moderate 25)
  • Regular Psychological Monitoring: Implement frequent psychological follow-ups to assess and adjust treatment strategies effectively. (Evidence: Expert opinion 25)
  • Family Involvement: Encourage family participation in the treatment process to provide support and understanding. (Evidence: Expert opinion 25)
