Traumatic bronchopleural fistula

Last edited: 4/22/2026

Overview

Traumatic bronchopleural fistulas are abnormal communications between a bronchus and the pleural space, often resulting from blunt or penetrating chest trauma, leading to air leakage into the pleural cavity and potential respiratory complications.

Diagnosis

  • Clinical Presentation: Symptoms include dyspnea, cough with air leak, chest pain, and recurrent pneumothorax 12.
  • Imaging: Chest CT with high-resolution techniques often reveals the fistula and associated abnormalities 12.
  • Bronchoscopy: Essential for direct visualization and localization of the fistula 12.
  • Pleural Fluid Analysis: May show air bubbles indicative of air leak 12.
  • Management

  • Primary Treatment:
  • - Endovascular Stent Placement: Effective for sealing the fistula, particularly in complex cases 125. - Surgical Repair: Indicated for large or complex fistulas not amenable to endovascular techniques 1219.
  • Adjunctive Measures:
  • - Pleural Drainage: To manage pneumothorax and prevent lung collapse 12. - Antibiotics: Prophylactic use in cases with signs of infection 12.

    Special Populations

  • Pediatrics: Endovascular techniques are increasingly favored for their minimally invasive nature 1.
  • Elderly: Surgical interventions may be more challenging due to comorbid conditions; endovascular approaches are preferred when feasible 12.
  • Comorbidities: Patients with significant comorbidities may require tailored approaches balancing invasiveness and efficacy 12.
  • Key Recommendations

  • Endovascular stent placement should be considered first-line for traumatic bronchopleural fistulas, especially when feasible, to minimize invasiveness and optimize outcomes (Evidence: Moderate 125).
  • Surgical intervention is recommended for complex or recurrent fistulas that do not respond to endovascular methods (Evidence: Moderate 119).
  • Regular monitoring and pleural drainage are essential adjuncts to manage complications such as pneumothorax (Evidence: Expert opinion).
  • Prophylactic antibiotics should be considered in patients at high risk for infection following the repair procedure (Evidence: Expert opinion).
