Autoimmune hepatitis

Last edited: 4/14/2026

Overview

Autoimmune hepatitis (AIH) is a chronic inflammatory liver disease characterized by immune-mediated damage to hepatocytes, leading to potential cirrhosis and liver failure if untreated 23.

Diagnosis

  • Clinical Presentation: Variable, from asymptomatic to acute liver failure 2.
  • Laboratory Findings: Elevated liver enzymes, hypergammaglobulinemia, presence of autoantibodies (e.g., ANA, SMA, LKM1) 911.
  • Histological Features: Necroinflammatory lesions in liver biopsy 9.
  • Exclusion Criteria: Viral hepatitis, metabolic disorders, and other causes of liver disease 9.
  • Simplified Criteria: Sex, age, autoantibodies, immunoglobulins, absence of viral hepatitis, and histology 21.
  • Management

  • First-Line Treatment: Corticosteroids (e.g., prednisolone) with or without azathioprine 2313.
  • Adjunctive Therapies: For inadequate response or intolerance, consider second-line agents like mycophenolate mofetil, calcineurin inhibitors (e.g., tacrolimus), or budesonide 13.
  • Monitoring: Regular assessment of liver function tests, autoantibody levels, and liver histology 117.
  • Dose Adjustment: Tailor immunosuppressive therapy based on response and side effects 15.
  • Special Populations

  • Pediatrics: Recognize two types based on autoantibodies (AIH-1 and AIH-2) 14.
  • Pregnancy: Management requires careful consideration of immunosuppressive therapy risks and benefits 25.
  • Comorbidities: Increased risk of Clostridium difficile infection in patients with AIH 23.
  • Key Recommendations

  • Initiate Corticosteroids for Confirmed AIH Diagnosis: Start with prednisolone, typically initiating at 20-40 mg/day, adjusting based on response (Evidence: Strong 23).
  • Add Azathioprine for Maintenance Therapy: Combine with corticosteroids to reduce steroid dependency and improve long-term outcomes (Evidence: Strong 23).
  • Consider Second-Line Agents for Refractory Cases: Evaluate and switch to agents like mycophenolate mofetil or calcineurin inhibitors for inadequate response or intolerance to first-line therapy (Evidence: Moderate 13).
  • Regular Monitoring Essential: Include periodic liver function tests, autoantibody levels, and clinical assessment to guide therapy adjustments (Evidence: Moderate 117).
  • Special Attention in Pediatric Patients: Tailor diagnosis and management considering autoantibody types specific to pediatric AIH (Evidence: Moderate 14).
