Healthcare associated pertussis

Last edited: 4/23/2026

Overview

Healthcare-associated pertussis (HA-pertussis) refers to pertussis infections acquired within healthcare settings, posing significant risks to vulnerable patient populations, particularly neonates and immunocompromised individuals. 7827

Diagnosis

  • Clinical symptoms include prolonged cough, paroxysms, and post-tussive vomiting.
  • Diagnostic confirmation through nasopharyngeal swab PCR testing or culture.
  • Serology may be useful for retrospective diagnosis or in cases with atypical presentations. 727
  • Management

  • First-line treatment: Antibiotic therapy, typically with azithromycin, erythromycin, or clarithromycin for 5-7 days. 7
  • Adjunctive measures: Isolation of infected individuals, contact tracing, and chemoprophylaxis for exposed close contacts.
  • Supportive care: Focus on symptom management and monitoring for complications, especially in neonates and immunocompromised patients. 727
  • Special Populations

  • Pediatrics: Neonates are particularly vulnerable; early diagnosis and isolation are critical to prevent outbreaks. 7
  • Elderly and immunocompromised: Require close monitoring for severe complications and prolonged antibiotic therapy may be necessary. 727
  • Key Recommendations

  • Implement strict infection control measures, including isolation protocols and contact tracing, to prevent HA-pertussis spread. (Evidence: Moderate 7)
  • Initiate prompt antibiotic therapy with macrolides or tetracyclines for confirmed cases to reduce transmission and severity. (Evidence: Moderate 7)
  • Provide chemoprophylaxis to close contacts, especially in high-risk settings like neonatal units, to prevent secondary infections. (Evidence: Moderate 7)
