Cardiorespiratory failure

Last edited: 4/13/2026

Overview

Cardiorespiratory failure involves critical impairment of both cardiac and respiratory systems, often leading to multiorgan dysfunction and potentially life-threatening conditions requiring intensive care interventions. 281130

Diagnosis

  • Clinical Presentation: Symptoms include severe dyspnea, hypoxemia, tachycardia, hypotension, and altered mental status. 2830
  • Diagnostic Tests:
  • - Imaging: Chest X-ray, CT scan to assess lung and cardiac involvement. - Laboratory Tests: Blood gas analysis, complete blood count, cardiac biomarkers, renal function tests. - Monitoring: Continuous hemodynamic monitoring, arterial blood gases. 281130
  • Grading: Utilize scoring systems like Sequential Organ Failure Assessment (SOFA) to quantify organ dysfunction severity. 230
  • Management

  • First-Line Treatments:
  • - Mechanical Ventilation: For respiratory failure, ensuring adequate oxygenation and ventilation. 2830 - Inotropic Support: For hemodynamic instability, using agents like dobutamine or milrinone. 230
  • Adjunctive Therapies:
  • - Extracorporeal Membrane Oxygenation (ECMO): For severe respiratory failure refractory to conventional ventilation. 2830 - Multimodal Blood Purification: In cases of intoxication or severe metabolic disturbances. 331 - Renal Replacement Therapy: For acute kidney injury associated with multiorgan failure. 331
  • Specific Interventions:
  • - Argon Therapy: Experimental use to mitigate ischemia-reperfusion injury in certain contexts. 19 - Plasma Exchange: For toxin-related multiorgan failure, particularly in cases of drug intoxication. 4151

    Special Populations

  • Pediatrics: Sedation management critical during radiological procedures; careful monitoring for paradoxical reactions. 44
  • Elderly: Increased susceptibility to multiorgan failure due to comorbid conditions; tailored hemodynamic support necessary. 30
  • Comorbidities: Patients with pre-existing respiratory or cardiac conditions may require more aggressive monitoring and intervention. 30
  • Key Recommendations

  • Utilize ECMO as a bridge to recovery or transplantation in patients with severe cardiorespiratory failure refractory to conventional ventilation. (Evidence: Strong 28)
  • Implement multimodal blood purification therapies in cases of severe intoxication contributing to multiorgan failure. (Evidence: Moderate 331)
  • Employ continuous hemodynamic monitoring and adjust inotropic support based on real-time data to manage hemodynamic instability effectively. (Evidence: Moderate 230)
  • Consider plasma exchange for patients with multiorgan failure secondary to drug toxicity, particularly valproic acid intoxication. (Evidence: Weak 4151)
  • Tailor management strategies in elderly patients by closely monitoring for and addressing comorbid conditions that exacerbate multiorgan dysfunction. (Evidence: Expert opinion)
