Juvenile idiopathic arthritis

Last edited: 4/23/2026

Overview

Juvenile idiopathic arthritis (JIA) is a group of chronic inflammatory arthritis conditions affecting individuals under 16 years of age, encompassing various subtypes including oligoarticular, polyarticular, systemic, and enthesitis-related arthritis. 1815

Diagnosis

  • Key Diagnostic Criteria: Persistent arthritis in one or more joints for at least six weeks, absence of other specific arthritic conditions. 11314
  • Recommended Tests:
  • - Laboratory tests: ESR, CRP, RF, ANA, anti-CCP (where applicable). 11315 - Imaging: Radiographs, MRI for joint damage assessment. 118
  • Classification Systems: ILAR and PRINTO criteria for subtype classification; PRINTO criteria show varying overlap with ILAR. 131418
  • Management

  • First-Line Treatments:
  • - Oligoarticular JIA: Nonsteroidal anti-inflammatory drugs (NSAIDs), intra-articular corticosteroid injections. 1222 - Polyarticular JIA: Methotrexate, other conventional DMARDs. 1815 - Systemic JIA: Corticosteroids, IL-1 inhibitors (e.g., anakinra, canakinumab). 192021
  • Adjunctive Treatments:
  • - Biologic DMARDs: TNF inhibitors (e.g., adalimumab, etanercept), IL-6 inhibitors (e.g., tocilizumab). 44451 - Physical therapy, occupational therapy, and orthotics for joint function and mobility. 115

    Special Populations

  • Pediatrics: Tailored dosing and monitoring for growth and development; frequent assessments for complications like uveitis. 1152630
  • Transition to Adult Care: Structured transition programs to ensure continuity of care; mental health screening recommended. 202258
  • Key Recommendations

  • Initiate early aggressive treatment with DMARDs for polyarticular and systemic JIA to control disease activity. (Evidence: Strong 1815)
  • Regular monitoring for uveitis in all JIA subtypes, especially those with positive ANA or RF. (Evidence: Moderate 2642)
  • Implement structured transition programs for adolescents moving to adult rheumatology care to maintain optimal outcomes. (Evidence: Expert opinion 2022)
  • Use of intra-articular corticosteroid injections for localized joint inflammation in oligoarticular JIA. (Evidence: Moderate 222)
  • Prompt escalation to biologic DMARDs in cases of inadequate response to conventional DMARDs. (Evidence: Moderate 444)
