Ischemic heart disease

Last edited: 4/23/2026

Overview

Ischemic heart disease (IHD) encompasses conditions where reduced blood flow to the heart muscle leads to myocardial ischemia, often due to atherosclerotic plaque formation in coronary arteries, causing significant morbidity and mortality globally.

Diagnosis

  • Clinical Symptoms: Typical angina, atypical chest pain, dyspnea 18.
  • Electrocardiography (ECG): ST-segment changes, T-wave inversions, new-onset arrhythmias 19.
  • Noninvasive Imaging: Echocardiography, myocardial perfusion imaging, coronary CT angiography 1028.
  • Invasive Coronary Angiography: Definitive for obstructive CAD, but also useful in INOCA for collateral assessment 48.
  • Functional Tests: Exercise stress testing, pharmacologic stress testing with imaging 18.
  • Management

  • Lifestyle Modifications: Smoking cessation, dietary changes (reduced sodium intake), increased physical activity 89.
  • Pharmacotherapy:
  • - Antiplatelet Agents: Aspirin 1. - Statins: To lower cholesterol levels 1. - Beta-Blockers: For symptom relief and reducing mortality 1. - ACE Inhibitors/ARBs: In heart failure or post-MI to improve outcomes 1. - Diuretics: For heart failure management 1.
  • Revascularization:
  • - PCI: For significant stenosis 1. - CABG: In selected cases with multivessel disease 14.
  • Secondary Prevention: Cardiac rehabilitation programs to enhance physical activity and quality of life 6667.
  • Special Populations

  • Pregnancy: Limited specific guidance; focus on risk stratification and conservative management 44.
  • Elderly: Tailored rehabilitation programs and careful consideration of comorbidities 64.
  • Comorbidities: Hypertension management crucial; consider specific guidelines like ACC/AHA 2024 perioperative recommendations 59.
  • Key Recommendations

  • Implement comprehensive cardiovascular prevention programs, including lifestyle modifications and pharmacotherapy, to reduce risk factors 1 (Evidence: Strong).
  • Utilize coronary CT angiography and functional testing for accurate diagnosis, especially in patients with INOCA 1048 (Evidence: Moderate).
  • Incorporate cardiac rehabilitation for improved functional capacity and quality of life in patients with IHD 6667 (Evidence: Strong).
  • Tailor management strategies for elderly patients, emphasizing rehabilitation and symptom management 64 (Evidence: Moderate).
  • Optimize hypertension control as part of secondary prevention strategies to mitigate cardiovascular risk 9 (Evidence: Strong).
