Plasmacytoma

Last edited: 4/14/2026

Overview

Plasmacytomas are rare neoplasms characterized by monoclonal plasma cell proliferation, often localized extramedullaryy outside the bone marrow. They can occur in various locations including the ureter, gastrointestinal tract, skin, and other organs, with clinical presentations varying based on site involvement 134678.

Diagnosis

  • Clinical Presentation: Symptoms depend on the location; e.g., hematuria and hydronephrosis for ureter involvement 1, dysphagia or dyspnea for upper aerodigestive tract involvement 3, skin lesions for cutaneous plasmacytomas 4.
  • Biopsy and Histopathology: Essential for diagnosis, showing monoclonal plasma cell infiltration 1245678.
  • Imaging: CT, MRI, or ultrasound to localize the tumor and assess extent 168.
  • Laboratory Tests: Serum protein electrophoresis and immunofixation to rule out multiple myeloma 15.
  • Infection Screening: Consider Helicobacter pylori testing in gastric plasmacytomas 2.
  • Management

  • Surgical Resection: Primary treatment for localized extramedullary plasmacytomas 168.
  • Radiation Therapy: Often used postoperatively to reduce recurrence risk 18.
  • Antibiotics: For associated infections, such as Helicobacter pylori eradication therapy in gastric plasmacytomas 2.
  • Monitoring: Regular follow-up with imaging and laboratory tests to detect recurrence 18.
  • Special Populations

  • Comorbidities: Consider Helicobacter pylori infection in gastric plasmacytoma management 2.
  • Elderly: Surgical and radiation risks need careful assessment due to comorbidities 168.
  • Key Recommendations

  • Surgical resection is recommended for localized extramedullary plasmacytomas to achieve complete removal and improve prognosis (Evidence: Strong 168).
  • Postoperative radiation therapy should be considered to reduce the risk of recurrence (Evidence: Moderate 18).
  • Screen for and treat associated infections, such as Helicobacter pylori in gastric plasmacytomas, to facilitate complete regression (Evidence: Moderate 2).
  • Regular follow-up with imaging and laboratory assessments is crucial for monitoring disease recurrence (Evidence: Expert opinion 18).
