Upper urinary tract infection

Last edited: 4/14/2026

Overview

Upper urinary tract infection (UTI) involves infections of the kidney (pyelonephritis) and ureter, often presenting with flank pain, fever, and hematuria. 7

Diagnosis

  • Clinical Presentation: Flank pain, fever, dysuria, hematuria 7
  • Laboratory Tests: Urinalysis (pyuria, bacteriuria, hematuria), blood cultures 17
  • Imaging: Ultrasound, CT scan to assess for structural abnormalities or complications 17
  • Urine Culture: Essential for identifying causative organisms and guiding antibiotic therapy 17
  • Management

  • Antibiotics: Initial broad-spectrum coverage followed by targeted therapy based on culture results (e.g., fluoroquinolones, cephalosporins) 17
  • Supportive Care: Hydration, pain management, monitoring for complications like sepsis 17
  • Surgical Intervention: Considered for refractory cases, obstruction, or abscess formation 17
  • Special Populations

  • Elderly: Higher risk of complications; careful monitoring and tailored antibiotic therapy 17
  • Comorbidities: Presence of diabetes or immunosuppression may necessitate longer treatment durations and closer follow-up 17
  • Key Recommendations

  • Initiate Empiric Antibiotic Therapy Based on clinical suspicion and local resistance patterns (Evidence: Moderate) 17
  • Perform Urine Culture and Sensitivity Testing To guide definitive antibiotic therapy (Evidence: Strong) 17
  • Consider Imaging For patients with recurrent infections, atypical presentations, or complications (Evidence: Moderate) 17
