Renal failure syndrome

Last edited: 4/23/2026

Overview

Renal failure syndrome encompasses various conditions leading to impaired kidney function, often necessitating renal replacement therapies such as dialysis or transplantation. It can result from acute or chronic kidney injury, with significant implications for cardiovascular health and overall prognosis. 4244

Diagnosis

  • Clinical Symptoms: Fatigue, edema, changes in urination patterns, and signs of uremia.
  • Laboratory Tests: Elevated serum creatinine, blood urea nitrogen (BUN), and electrolyte imbalances.
  • Imaging: Ultrasound, CT, or MRI to assess kidney structure and identify complications like thrombosis.
  • Renal Function Tests: Estimated glomerular filtration rate (eGFR) to grade severity.
  • Urine Analysis: Proteinuria, hematuria, and casts indicative of specific pathologies.
  • Special Considerations: Resistivity index (RRI) in ultrasound for microcirculation assessment 23.
  • Management

  • Renal Replacement Therapy: Hemodialysis, continuous renal replacement therapy (CRRT) for acute cases.
  • Medications:
  • - Blood Pressure Control: Angiotensin-converting enzyme (ACE) inhibitors or angiotensin receptor blockers (ARBs) 33. - Electrolyte Management: Adjustment of calcium, phosphate, and potassium levels. - Infection Prevention: Prophylactic antibiotics for catheter-related infections.
  • Nutritional Support: Management of hyperphosphatemia and malnutrition 1070.
  • Cardiovascular Risk Reduction: Tadalafil for potential protection against multiple organ failure 30.
  • Special Populations

  • Pediatrics: Bilateral kidney absence requires careful management of hypotension and renin-angiotensin-aldosterone system (RAAS) involvement 1.
  • Elderly: Complex decision-making for dialysis initiation, considering comorbidities and functional status 48.
  • Comorbidities: Increased cardiovascular risk necessitates tailored management strategies 1424.
  • Key Recommendations

  • Initiate renal replacement therapy based on clinical symptoms and laboratory markers of kidney function (Evidence: Strong 42).
  • Use ACE inhibitors or ARBs for blood pressure control in patients without contraindications (Evidence: Strong 33).
  • Monitor and manage electrolyte imbalances, particularly hyperphosphatemia, through dietary and pharmacological interventions (Evidence: Moderate 1070).
  • Consider individualized approaches to dialysis in elderly patients, balancing quality of life and survival (Evidence: Expert opinion 48).
  • Implement strategies to prevent and manage catheter-related infections in dialysis patients (Evidence: Moderate 20).
