Shrinking lung syndrome

Last edited: 4/22/2026

Overview

Shrinking lung syndrome, also known as hypogenetic lung syndrome, encompasses congenital anomalies characterized by hypoplasia of a lung, often accompanied by systemic arterial supply and anomalous systemic venous drainage, sometimes referred to as scimitar syndrome 12.

Diagnosis

  • Key Diagnostic Criteria: Hypoplasia of a lung, systemic arterial supply to the affected lung, and anomalous systemic venous drainage mimicking a scimitar vein on imaging 2.
  • Recommended Tests:
  • - MRI: Utilize ECG-triggered turbo spin-echo, cine, velocity-encoded cine sequences, and contrast-enhanced MRA for comprehensive anatomic and functional evaluation 1. - Chest Radiography: Initial imaging to identify characteristic appearances of anomalous systemic veins 2. - Angiography: Essential for detailed visualization of anomalous venous drainage and arterial supply 2.

    Management

  • First-Line Treatments:
  • - Surgical Intervention: Typically required for definitive correction of systemic arterial supply and venous anomalies 2.
  • Adjunctive Treatments:
  • - Supportive Care: Focus on managing symptoms and complications, such as respiratory support and monitoring for infections 2.

    Special Populations

  • Pediatrics: Early surgical correction is often necessary to prevent long-term respiratory complications 2.
  • Comorbidities: Presence of additional anomalies like anomalous pulmonary veins may influence surgical complexity and approach 2.
  • Key Recommendations

  • Perform comprehensive MRI evaluation including functional and anatomic assessments for definitive diagnosis of hypogenetic lung syndrome (Evidence: Moderate 1).
  • Angiography is crucial for identifying all components of the syndrome, including systemic arterial supply and venous anomalies (Evidence: Moderate 2).
  • Surgical correction should be considered early in pediatric patients to address systemic arterial supply and anomalous venous drainage (Evidence: Expert opinion 2).
